The number of heart failure patients benefiting from telemedicine technology is growing rapidly. With the inCareNet HF platform, partners SEMDATEX GmbH, BIOTRONIK Germany and GETEMED AG offer powerful, guideline-based telemonitoring for the modern care of chronically ill people.
More than 10,000 patients are already registered on the inCareNet HF platform, whose implant and sensor data is consolidated in a central, QSV-compliant dashboard – supported by advanced AI for efficient evaluation and patient-specific analysis.
Telemonitoring for heart failure has been part of standard care in Germany since 2022 and enables continuous care according to the highest quality standards. SEMDATEX's infrastructure ensures cross-sector networking between clinical teams, registered cardiologists and telemonitoring centres. Hospital and practice staff receive all relevant vital parameters – blood pressure, weight, pulse, ECG – as well as implant data (ICD, CRT devices) and daily self-assessments from patients in a clear, digital patient file. Encrypted transmission and mobile device management ensure data security at all times.
Partners BIOTRONIK Germany (telemonitoring of implants), GETEMED (external sensors) and SEMDATEX are pooling their expertise to significantly improve patient care and quality of life. The technical and organisational integration of the platform meets all the requirements of the G-BA quality assurance agreement and supports medical facilities in establishing a telemonitoring centre (TMZ) in accordance with the latest standards. InCareNet HF thus optimises the care of people with advanced heart failure. Comprehensive data usage, intuitive dashboards and AI-supported evaluation enable timely and needs-based medical intervention.
